At Midland Eye we aim to deliver a high quality service to all our patients at all times so your feedback is very important to us. It helps us monitor how well we are doing and helps us to achieve the highest standards of care.
When you attend the clinic, we will ask you for your feedback towards the end of your visit. We would be grateful for your time in completing our feedback forms and welcome feedback, whether positive or negative.
This questionnaire relates to your appointment and experience with us. It is completely anonymous and should only take a few minutes to complete.
We do hope not, but if you are unhappy with any aspect of your care, please speak to a member of staff straight away so that we can do our best to put things right for the remainder of your time with us.
